Анализ уровня качества жизни (КЖ) у больных с варикозной болезнью нижних конечностей (ВБНК) в госпитальном и отдаленном периоде наблюдения после эндовазальной лазерной облитерации (ЭВЛО) и комбинированной флебэктомии (КФ). Материалы и методы исследования. В данное проспективное исследование включено 408 пациентов с ВБНК прооперированных за период с 2013 - 2017. В зависимости от стратегии лечения больные были распределены на две группы: группа 1 - пациентам проводилась ЭВЛО. В группу вошли 257 пациентов, что составило 63% от включенных в исследование; группа 2 - пациентам проводилась КФ. В группу вошел 151 пациент, что составило 37% от включенных в исследование. На предоперационном этапе был определен спектр клинических проявлений ХВН в соответствии с клинической, этиологической, анатомической и патофизиологической системой классификации (СЕАР). Для оценки КЖ использовалось два типа шкал: Chronic Venous Diseasequality of life Questionnaires (CIVIQ) и SF-36 Health Status Survey. Результаты и обсуждение. Через 2 года после проведенного леченияпо данным шкалы CIVIQ отмечается статистически значимая динамика КЖ при применении обоих тактик лечения. При межгрупповом сравнении показателей через два года после проведенного лечения выявляется статистически значимое различие (р=0,008), что обусловлено наличием феномена реканализации (рецидива) по данным УЗИ и частичным возвратом симптоматики ХВН у больных группы КФ. При межгрупповом сравнении результатов опроса по шкале SF-36 через 2 года лечения определяется ряд значимых различий. Так группа ЭВЛО показала более высокие уровни качества жизни по большинству показателей. Заключение. Таким образом, обе стратегии лечения статистически значимо поднимали уровень качества жизни пациентов в отдаленном послеоперационном периоде наблюдения. Однако ЭВЛО показала большую эффективность в динамике данного показателя относительно пациентов с КФ, что, вероятнее всего было обусловлено наличием реканализации у каждого второго пациента второй группы через два года наблюдения.</p></abstract><trans-abstract xml:lang="en"><p>Aim. To analyze the life quality of patients with varicose veins of lower limbs (VVLL) during hospital and longterm observation after endovenous laser obliteration (EVLO) and combinedphlebectomy (CF). Material and method. This prospective study included 408 patients with varicose veins of lower limbs who underwent surgery for the period from 2013 - 2017. Based on treatment strategy, patients were divided into two groups: the first group included 257 (63%) patients that underwent EVLO; patients of the second group included patients that underwent CF in the number of 151 (37%) patients. At the preoperative stage, the spectrum of clinical manifestations of CVI was determined based on the clinical, etiological, anatomical and pathophysiological classification system (CEAP). To assess the life quality, two types of scales were used: Chronic Venous Disease quality of life Questionnaires (CIVIQ) and SF-36 Health Status Survey. Results and discussion. 2 years after the treatment, according to the CIVIQ scale, statistically significant life quality dynamics using both treatment tactics were observed. Comparison of the indicators two years after the treatment revealed a statistically significant difference (p = 0.008) due to the presence of the phenomenon of recanalization (relapse) according to ultrasound and a partial return of CVI symptoms in patients of the CF group. A comparison of the survey results between groups according to the SF-36 scale in 2 years after the treatment showed significant differences. tTe EVLO group showed higher levels of life quality in most indicators. Conclusion. Thus, both treatment strategies statistically significantly increased the life quality of patients in the long-term postoperative follow-up period.
