Изучение причин материнской смертности при летальных SARS-COV-2-ассоциированных пневмониях во время гестации с целью оптимизации оказания медицинской помощи данной категории беременных.   Материалы и методы исследования. Было обследовано 70 беременных женщин со среднетяжёлым, тяжёлым и критическим течением SARS-CoV-2-ассоциированных пневмоний. Объём исследования соответствовал стандарту обследования беременных при коронавирусной инфекции SARS-CoV-2. От общего числа обследованных7 случаев (1 %) закончились летально.</p><p>   Результаты исследования и их обсуждение. Анализ организационных аспектов ведения выявил дефекты организации оказания медицинской помощи беременным при коронавирусной инфекции SARS-CoV-2 на амбулаторном и стационарном уровне. Причинами поздней госпитализации стали: самолечение беременных в домашних условиях в течение 3-х суток, лечение амбулаторным звеном в течение 7-10 суток, первичная госпитализация была осуществлена в непрофильные стационары. Нарушение маршрутизации беременных возникла в 2 (28,6 %) случаях. В стационаре недооценка состояния способствовала затягиванию времени родоразрешения в 5 (71,4 %) случаях.   Вывод. Организационными дефектами оказания медицинской помощи данному контингенту больных явились: поздняя госпитализация в профильный стационар, поздняя диагностика тяжёлых и осложненных форм, позднее родоразрешение, как единственный метод, способный изменить смертельный исход осложнений при летальных SARS-CoV-2-ассоциированных пневмониях.</p></abstract><trans-abstract xml:lang="en"><p>   Aim. To study the causes of maternal mortality in fatal SARS-COV-2-associated pneumonia during gestation in order to optimize medical care for this category of pregnant women.   Material and methods. 70 pregnant women with moderate, severe and critical course of SARS-CoV-2 were examined. The extent of study corresponded to the standard of examination of pregnant women with SARS-CoV-2 coronavirus infection. Out of all cases 7 (1 %) were lethal.   Results and discussion. Analysis of organizational aspects of management revealed defects in the organization of medical care for pregnant women with SARS-CoV-2 coronavirus infection at the outpatient and inpatient levels. The reasons for late hospitalization were: self-treatment of pregnant women at home within 3 days, outpatient treatment within 7-10 days, primary hospitalization carried out in non-specialized hospitals. Routing breaches in pregnant women occurred in 2 (28.6 %) cases. Underestimation of the condition in the hospital contributed to delayed delivery in 5 (71.4 %) cases.   Conclusion. Breaches in the organizational aspects of medical management of lethal SARS-CoV-2-associated pneumonia correspond with late hospitalization, late diagnosis and underestimation of the condition contributed to the delaying of delivery.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>вирусная пневмония</kwd><kwd>беременность</kwd><kwd>коронавирусная инфекция</kwd><kwd>SARS-CoV-2</kwd><kwd>материнская смертность</kwd></kwd-group><kwd-group xml:lang="en"><kwd>viral pneumonia</kwd><kwd>pregnancy</kwd><kwd>coronavirus infections</kwd><kwd>SARS-CoV-2</kwd><kwd>respiratory viral infection</kwd><kwd>maternal mortality</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Ишанходжаева Ф.
