Изучить показатели стандартной коагулограммы и теста тромбодинамики в нативной плазме и в тесте с фосфолипидами в зависимости от активности ревматоидного артрита и проводимой терапии.</p><p>Материалы и методы исследования. Обследованы 28 больных ревматоидным артритом и 8 сопоставимых по возрасту и полу здоровых человек группы контроля: 11 пациентов с высокой активностью ревматоидного артрита (DAS28&gt;5,2) (1 группа), 9 больных с низкой и умеренной активностью ревматоидного артрита (DAS28&lt;5,2) (2 группа) и 8 больных ревматоидным артритом, получавших антикоагулянты (3 группа). Результаты исследования и их обсуждение. В стандартных тестах свёртывания повышение фибриногена чаще встречалось в группах больных ревматоидным артритом: в 1-й - 5/11(45%), во 2-й - 3/9(33%) и в 3-й – 3/8(38%) пациентов, в контроле фибриноген был в норме, р&gt;&lt;0,05. Повышение РФМК отмечалось у всех больных в трех группах (100%) и только у двоих в группе контроля (25%), p&gt;&lt;0,05. В тестах тромбодинамики средняя плотность сгустка была достоверно выше в группах больных ревматоидным артритом, чем в контрольной: 26811, 25437 и 24740 против 20631 усл.ед. в 1, 2, 3 группах и контрольной соответственно, p&gt;&lt;0,01. В группе 3 отмечалась более выраженная задержка роста сгустка 1,4 минуты, тогда как без антикоагулянтов задержка составляла 1,1-1,2 минуты, р&gt;&lt;0,05 во всех случаях. Спонтанные сгустки выявлялись только у пациентов: у 3-х в 1-й группе и по 1-му во 2-й и 3-й группах. &gt;˂ 5,2) (2 группа) и 8 больных ревматоидным артритом, получавших антикоагулянты (3 группа).</p><p>Результаты исследования и их обсуждение. Частота нормо- и гиперкоагуляции в группах больных и контрольной не различалась.</p></sec><sec><title>Заключение</title><p>Заключение. По данным теста тромбодинамики у больных РА имеет место гиперкоагуляция, что свидетельствует о состоянии тромботической головности. Тест тромбодинамики может быть использован для выявления риска тромбоза и персонификации терапии у больных ревматоидным артритом. </p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To study the indices of standard coagulogram and thrombodynamics test in native plasma and in the test with phospholipids in relation to the activity of rheumatoid arthritis (RA) and the ongoing therapy.</p></sec><sec><title>Material and methods</title><p>Material and methods. In patients with anticoagulants, lag time (1.4 minutes) was longer than without anticoagulants (1.1-1.2 minutes), with p&gt;&lt;0.05 in all cases. Spontaneous clots were detected only in patients: 3 in groups 1 and 1 each in groups 2 and 3. Hypocoagulation was detected only in group 3 in 1/8(12%) patients, and thrombotic readiness status was detected in groups 1 and 2: in 2/11(18%) and 1/9(11%) patients, respectively. The frequency of normal- and hypercoagulation did not differ between the patient and control groups. Conclusion. According to the thrombodynamics test, hypercoagulability is present in RA patients. Thrombodynamics test can be used to identify the risk of thrombosis and to individualize therapy in RA patients.
