Оптимизация лечения травм печени через анализ результатов операций.</p><p>Материал и методы исследования. Проведен ретроспективный анализ данных 98 пациентов, перенесших реконструктивные операции на сосудах печени при сочетанной травме с 2018 по 2023 годы. Пациенты были разделены на три группы в зависимости от типа сосудистых повреждений печени: тип A (30 пациентов), тип B (40 пациентов), тип C (28 пациентов). Для диагностики использовались КТ, ангиография, лабораторные анализы. Операции проводились с применением микрохирургических техник и инновационных материалов. Результаты исследования и их обсуждение. Средняя длительность операции увеличивалась с типом повреждений: для типа A — 1,81 часа, для типа B — 2,84 часа, для типа C — 3,04 часа. Уровень кровопотери также был выше у пациентов с типами B и C. Частота осложнений и повторных операций была выше у пациентов с типом C (20,84% осложнений и 18,36% повторных операций), в то время как тип A имел наименьшие показатели осложнений и повторных вмешательств. Выживаемость была самой высокой у пациентов с типом A (99,11%), и самой низкой у пациентов с типом C (82,53%).</p></sec><sec><title>Заключение</title><p>Заключение. Реконструктивно-восстановительные операции на сосудах печени при сочетанной травме демонстрируют различные результаты в зависимости от тяжести повреждений. Пациенты с артериальными повреждениями (тип A) показывают лучшие операционные и долгосрочные результаты по сравнению с пациентами с венозными и смешанными повреждениями (типы B и C). Улучшение методов диагностики и хирургического вмешательства способствуют снижению частоты осложнений и повышению выживаемости.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim. To optimize the treatment of liver trauma by analyzing surgical outcomes.</p></sec><sec><title>Materials and Methods</title><p>Materials and Methods. A retrospective analysis was conducted on data from 98 patients who underwent reconstructive liver vascular surgeries for combined trauma between 2018 and 2023. Patients were categorized into three groups based on the type of liver vascular injury: Type A (30 patients), Type B (40 patients), and Type C (28 patients). Diagnostic methods included computed tomography (CT) scans, angiography, and blood tests. Surgeries were performed using microsurgical techniques and innovative materials.</p></sec><sec><title>Results and Discussion</title><p>Results and Discussion. The average duration of surgery increased with the severity of liver injury: Type A surgeries averaged 1.81 hours, Type B averaged 2.84 hours, and Type C averaged 3.04 hours. Blood loss was also significantly higher in patients with Type B and Type C injuries. Complication and reoperation rates were highest among Type C patients (20.84% complication rate and 18.36% reoperation rate), while Type A patients had the lowest rates. Survival rates were highest in Type A patients (99.11%) and lowest in Type C patients (82.53%).</p></sec><sec><title>Conclusion</title><p>Conclusion. The results indicate that reconstructive liver vascular surgeries for combined trauma yield outcomes that vary by the type of vascular injury. Arterial injuries (Type A) are associated with more favorable prognoses, whereas venous and mixed injuries (Types B and C) present greater challenges.
