Многочисленные мультицентровые исследования доказали, что инфицированность ВПЧ 16 и ВПЧ 18 среди возрастных групп, прививаемых в рамках плановой вакцинации, снизилась на 90%, заболеваемость генитальными кондиломами среди молодых мужчин и женщин снизилась на 88%, заболеваемость цервикальной интраэпителиальной неоплазии тяжёлой степени (CIN2+) снизилась на 70%. Заболеваемость инвазивной формой рака шейки матки (РШМ) во многих странах, где девочки подростки были привиты, снизилась, а среди женщин репродуктивного возраста приблизились к нулевой отметке. Глобальный комитет по безопасности вакцина в 2017 г. опубликовал заявление о безопасности ВПЧ вакцин и подтвердил рекомендации ВОЗ по использованию во всех странах мира.Заключение: включение вакцинации против ВПЧ в календарный план прививок является важным шагом на пути элиминации рака шейки матки в Таджикистане. Четырёхвалентная вакцина Гардасил, которой будут прививать девочек 10-летнего возраста, обладает высокой эффективностью выше 95-97% и клинической безопасностью.</p></abstract><trans-abstract xml:lang="en"><p>Objective: to review evidence from high-income countries on HPV vaccination effectiveness and clinical safety to inform the planning of an immunization campaign in the Republic of Tajikistan.Material and methods. A literature search was conducted to identify clinical and epidemiological studies on the effectiveness and safety of human papillomavirus (HPV) vaccines administered within the past 10–15 years (MEDLINE/ PubMed, WHO sources, and other relevant databases).Results. HPV vaccines are highly effective (97-100%) at preventing cervical precancer and cancer. Numerous multicenter studies have shown that HPV 16 and HPV 18 infection declined by 90% in age cohorts targeted by routine vaccination. The incidence of anogenital warts among young men and women has decreased by 88%, and the incidence of severe cervical intraepithelial neoplasia (CIN2+) has decreased by 70%. In many countries that introduced the vaccination of adolescent girls, the incidence of invasive cervical cancer has fallen, and in some settings, it has approached near-zero levels among women of reproductive age. In 2017, the WHO Global Advisory Committee on Vaccine Safety issued a statement affirming the safety of HPV vaccines and reaffirming WHO recommendations for their worldwide use.Conclusion. Inclusion of HPV vaccination in the immunization schedule is an important step towards eliminating cervical cancer in Tajikistan.
