Analysis of regulatory documents to strengthen the HPV vaccination system and early detection of cervical cancer in the Republic of Tajikistan

Abstract
Objective: o analyze the regulatory framework of the Republic of Tajikistan in the field of oncology in order to optimize the national model for early detection of cervical cancer.
Material and methods: A retrospective analysis was performed on 20 regulatory acts, national strategies, ministerial decrees, and international health system reviews from 2016 to 2025, using systemic and comparative descriptive methods.
Results: The study outlines the evolution of the national regulatory framework, from isolated pilot projects to a systemic approach embodied in the National Program for the Prevention and Control of Oncological Diseases for 2025–2029 (Decree No. 327). Another important step was Resolution No. 408, which regulated the launch of a universal HPV vaccination campaign for girls aged 10–14 in primary health care (PHC) facilities. At the clinical level, Decree No. 841 standardized cervical cytology using the Bethesda system (LSIL/HSIL). The authors substantiate the need for legislative consolidation of national digital monitoring algorithms through the Health Management Information System, which would improve the accessibility of preventive and diagnostic services for women facing financial and geographical barriers.
Conclusions: Tajikistan’s regulatory framework for the control of cervical cancer has been aligned with international WHO standards. Future efforts should focus on legally defining automated cross-level monitoring between primary care and specialized cancer centers.
